Routine appointments

To book an appointment, please fill out our online triage form. Our team assesses each enquiry and, depending upon your enquiry, will contact you to arrange an appointment with a suitable clinician or provide advice by text or phone. Need help with the form? Call our reception, and a team member will assist you in submitting the form for you by asking necessary questions and following the process you would be doing yourself online.

Your GP surgery should be your first point of contact if you are concerned about your health.
Appointments can take place face-to-face by phone, or video call, depending on your preference and clinical needs.
A range of healthcare professionals work at your practice and the wider community to help you get the right care when needed. This means a GP might see you, but you could also be seen by another trained professional, such as a practice nurse, community pharmacist, or physiotherapist.

Urgent appointments

Please fill out our online triage form . Our team assesses each enquiry and, depending on the urgency and type of enquiry, will contact you to arrange an appointment with a suitable clinician or provide advice by phone or text. Need help with the form? Call our reception, and a team member will assist you in submitting the form for you by asking the necessary questions.

Please note that we handle urgent and routine appointment requests through the same process. When you submit your inquiry, your appointment will be scheduled based on the urgency of your need and our current appointment availability.

 

If you are feeling overwhelmed and need urgent help because you are worried that you might harm yourself or someone else, contact your local mental health crisis line. Trained professionals are there to help you 24 hours a day, 365 days a year by calling Mental Health Direct free on 0800 995 1000

Cancelling your appointment

  • You can cancel any appointment you have scheduled in our practice via NHS App
  • Also, if your appointment is face-to-face, you receive a reminder SMS 24 hours before your appointment with an active cancellation link. You can cancel your appointment via that link.
  • Or you can submit the cancellation form via the link bellow.

Extended access

Planned care appointments with different clinicians such as advanced nurse practitioners, practice nurses or health care assistants are available 7 days a week, including in the evenings, on weekends and on bank holidays. These appointments can be booked online or over the phone.

Appointments out of hours 

If you need an urgent appointment, contact the practice during opening hours between Monday to Friday 8am to 6.30pm on 02085921771 or complete an online form.  

When the practice is closed, please ring NHS 111 or use NHS 111 online and they can help you with booking an urgent GP appointment. 

Routine and urgent appointments are provided in the evening and weekends at the following locations. You will be told which location you need to travel to when your appointment is booked: 

  • Barking Hospital Hub – Upney Ln, Barking IG11 9LX
  • Becontree Hub – 641-645 Becontree Ave, RM8 3HP
  • Broad Street Hub – Morland Rd, Dagenham RM10 9HU
  • Parsloes Hub – 2 Hobart Rd, Dagenham RM9 5NH

You should only attend A&E if you need immediate care for something very serious or life-threatening. If your condition isn’t life-threatening, in many cases, you could be seen faster by using other local services in your community. 

If you feel unwell or have a minor injury, your local pharmacy can also offer advice and some medicines. Many pharmacies are open late and at the weekends, and you do not need an appointment to be seen, Find your local Pharmacy

 

Home visits

Home visits are strictly for patients who are medically housebound, and requests for visits should be made as early as possible. Your GP will only visit you at home if they think your medical condition requires it and will also decide how urgently a visit is needed.

A community nurse may also visit you at home if your GP refers you.

A health visitor should also visit you at home if you have recently had a baby.
